Do not complete any portion of this form until that time as it will not save or transmit information entered into the form before this time. Forms should not be submitted to the Student Services Office. You will be notified electronically that your submission is received. Seat assignments will be made in accordance with the priority assignment process described on the seminar priority assigned system on the Registration web page. Students may access Banner after 5 p.m. on WEDNESDAY, April 2, 2008 to determine if they have been assigned a seat.
